How to file a complaint

Complete USDA Program Discrimination Complaint Form AD-3027, available through the USDA complaint process, any USDA office, or by calling (866) 632-9992. A letter may also include the complainant’s name, address, telephone number, and a sufficiently detailed description and date of the alleged action.
